Please submit the below documents to the ISS office before June 1(Sun)
(1) Application form (Please go to the Academic Information Systems to fill in the application form and print it out. The form should be duly signed by the original department Chair and the advising professor.)
(2) Transcript
(3) ISS personal information form
Ø Master program: Word
(4) (Required for the Ph.D. program) Certificate or relevant documents of language proficiency.
(5) Suggest confirming the guidance wishes with the professor of ISS before applying.
